To save time at your appointment check in you can download and complete your patient forms. If you have questions, we will be glad to assist you when you arrive.
Payment is due at time of service. If billing is needed Care Credit & Wells Fargo Financing is available, clients may apply for one of the following either online or at the hospital:
Appointments arriving more than 10 minutes late may need to be rescheduled or worked into the doctor’s schedule as time allows. After two no-show appointments, the client will be required to prepay for their examination prior to being seen. We require a 24 hour notice to change or cancel an appointment and for medication refills.
Because we do not have an adequate working relationship established with most internet/mail order pharmacies and because of the unknown conditions encountered in transport and storage, the unknown legal status of some pharmacies, we do not authorize or approve prescription requests sent to us by fax or telephone from internet/mail order pharmacies. If a client wishes to use an internet/mail order pharmacy we will issue a written prescription to the client which they may submit to the pharmacy of their choice, releasing Southern California Veterinary Dental Specialties from liability if the product is misbranded and/or causes harm.
